Elector’s Photo Identity Card is a most important document for Indian citizens. It is an Identification of people and also a residence proof. Without having of an EPIC, the citizen can not able to cast a vote in Elections. It issued by the Chief Electoral Officer of the respective states. In Madhya Pradesh State, the Chief Electoral Officer are responsible for issuing an Election Card and providing all the services related EPIC to the citizens. The citizen can download the necessary forms, enroll name as a voter, fill the online application, track the application status, Know your Polling Station/BLO, Check Electoral Rolls, Search your name in voter list etc through using the official web portal of CEO (MP). For Seeking Deletion of Own Name/Any Other Person’s Name in Electoral Roll (Madhya Pradesh) Any person, if want to claim for objection inclusion of the name of someone else due to Death/Shifting, so they can apply by filing an application in form-7. But the objector name should be included in the particular constituency. The citizen can also fill up the form in form-7 if he/she are seeking deletion of own name in voter list. The application form is available in the office of Electoral Registration Officer of the constituency. The citizen can download the application Form-7 in 2 languages either in English or Hindi, download it by clicking on the link as per convenience. Required Documents for Voter ID Card Deletion- Application in prescribed proforma (Form-7) Passport size colored photograph of applicant Voter’s Photo Identity Card which is to be deleted Objector’s Photo Identity card required if application filing for seeking deletion of any other person’s name. Photocopy of documentary proof of Identification of applicant like Aadhar Card, Driving License, Indian Passport, Ration Card etc. All documents photocopies must be duly attested by Gazetted Officers. Procedure to fill Form-7 for Objecting Inclusion of Name of Another Person in the Voter List (Madhya Pradesh State) First of all, the applicant will have to fill Assembly/Parliamentary Constituency in prescribed place. Then tick at one alternative among of 3 options- the first alternative is that object to the proposed inclusion of the name of the other person in electoral roll, the second option is that request the entry to name of the person is required to be deleted or the third option is that the request that entry relating to yourself is to be deleted from the voter list. Then fill the given below following details for the complete fill up the form. (1) Please Fill the Particulars of the applicant- Firstly enter the applicant’s Name and Surname Then enter Part number of the constituency Fill the Serial number of that part Write the Elector’s Photo Identity Card number in the next box (2) Kindly fill the Details of Person inclusion of whose name is objected to/whose entry to be deleted- Here the applicant needs to fill up that person's full name whose entry to be deleted. Then fill up Part number of the constituency and the Serial number of that part Also, fill up the Voter’s Photo Identity Card number (If Issued) In the next section, write the specific reason for objection/deletion of an entry in voter list- due to Shifting, due to Death or any other reason (Give Description). (3) Fill the Declaration Form in the end- Now the applicant will have to fill up self-declaration that the above-mentioned facts are correct and true to the best of your knowledge and belief. Please remember, giving an incorrect and false declaration is punishable under section 31 of the Representation of the People Act, 1950. Then enter the Place, Date, and Signature of Applicant. The rest of form will be filled by ERO of the constituency. Before submitting the application enclosed the supporting documents and put it in an envelope and send it to the Office of ERO. The applicant submits the application form by visiting the office of ERO/AERO or Booth Level Officer as well. While submission, you will get back an acknowledgment receipt from the officer. Keep the receipt for further assistance. After verification of the concerned officer, the entry will be deleted from the electoral roll shortly and you will be notified too. Note: - No person can be enrolled in more than one electoral roll, so while getting yourself enrolled at the new place of residence get your name deleted from the voter list of old assembly constituency.
