The Elector’s Photo Identity Card (EPIC) is an important document in India. It is well known as Voter ID Card. But in the case the original Voter ID Card is lost/damaged or theft then needs to apply for duplicate Voter/Election card as soon as possible. Form-001 Application for Duplicate Elector’s Photo Identity Card In case the Elector’s Photo Identity Card (EPIC) has lost/theft or damaged then the citizen need a duplicate Epic card. The citizen can apply for duplicate Voter ID Card by filling the Form-001. The application form-001 is easily available in the Chief Electoral Office. Eligibility Criteria- If any of citizen has lost, theft or damaged their Voter/Election card by mistake, can be eligible for apply a duplicate Voter ID card. But first they need to lodge a FIR in local Police Station and take the complaint slip for future assistance. Form-001 (For Duplicate Voter ID Card) The citizen of India can download the “Application Form-001” through the official web portal of Chief Electoral Officer or download it through the given link below. The following Procedure to fill Form-001 Firstly the applicant needs to fill their assembly/Election area name and number in required place. Then the applicant needs to fill Elector name, EPIC number, Mother’s/Father’s name, Gender, Date of birth etc. In the next section, fill the residence address like House/Door number, Street/Area, Village/Town, Post office, Pin code, Thana, District etc. Fill the reason why you apply for a Duplicate voter/election card. Now you will have to tick the option that how you want to receive a duplicate Voter/Election card among of 3 options- by post, by CSC center or by BLO. In the last, tick the option between you are returning your old/damaged Epic or have lost somewhere, if I will found then must be sent in the office on the same date. The last section (Issuance of Duplicate Voter/Election Card) will be filled by the Electoral Officer of your area.
