In the last section, please fill the declaration- Now you will have to declare that the facts as mentioned earlier and particulars are true and correct to the best of your knowledge and belief. Giving a false or incorrect declaration is punishable under section 31 of the Representation of the People Act, 1950. Then indicate the date from which you are residing at the given address, and lastly do you signature or thumb impression in the prescribed space. The rest of sections of application form will be filled by the Electoral Registration Officer of the constituency. Before submitting the form enclosed the relevant documents (All paper photocopy should be duly attested by Gazetted Officer).
